Essex Connect programme to feature at NHS Confed Virtual Annual Conference
Connect, the programme aimed at transforming care for older people by bringing together local authority and health partners across the county of Essex, is to be discussed at this year’s NHS Confed Virtual Annual Conference taking place from 15-17 June 2021.
The panel session, called, ‘Making integrated health and care a reality’ will feature a video which includes leaders and colleagues from across the Essex system and you can Click here for an exclusive preview. The video provides an excellent insight as to how leaders from across the system are seeing the impact of Connect. In addition, over the coming weeks a second edition of the video will be created which will include voices from the frontline
The true mark of a successful integrated care system will be its ability to quickly translate the multitude of changes coming through at a system level, into delivering better population health outcomes, better care and greater value for money for patients and service users.
The Confed session will explore this topic, and the panel discussion will explore the experience of this partnership from the perspective of different system partners (not least the impact of doing this during a pandemic); the immediate and longer-term benefits of the programme for the people of Essex and the system and how the work can provide a template for transformational change that could be replicated anywhere in the country.
