Anticoagulation clinic
Note: all hospitals in mid and south Essex will be reviewing all patients on warfarin attending hospital anticoagulant clinics with a view to switching suitable patients to a NOAC-first line being edoxaban.
Patients will receive their initial month’s supply of NOAC from the hospital, but GPs will be asked to continue to prescribe after the first month. Patients will be followed up by the hospital at 6 weeks to ensure no issues/concerns and then the patient will be discharged to the GP.
