Advance Care Planning and TEP guidance
With support from our local palliative care consultants, hospice’s and clinical leads we have designed a straightforward one page ‘Community Treatment Escalation Plan’ (TEP) for recording new conversations with patients and residents. This is designed for all clinical staff and care staff within the residential and community sector.
The Community Treatment Escalation Plan document for advance care planning has now been in place for over two months. Overall it has been received well and introduced across general practice and community settings. Following feedback from colleagues across the system, we have refined some elements of the current form and guidance to accompany it and these have also been passed through solicitors for review.
Could all organisations please look to embed this guidance into their local policies on advance care planning and ensure all who are using the CTEP are aware of the guidance and the need to follow accordingly.
We have designed a patient information sheet to accompany the conversations that we hope you will find helpful.
You do not need to replace any TEPs you have completed to date, this is a revision for new conversations. Please do ensure however that there is a review date for all CTEPs completed.
Please remember this isn’t to replace ACP documentation already in place for patients but for new conversations, and if you are comfortable using the PEACE document already established in Mid and South Essex, please continue to document using PEACE.
